Sports Physical Forms
Students must use their STUDENT School Google Account to access and complete registration.
All students interested in playing sports this coming school year, 2025-2026, should create a profile for adding their info, a current sports physical, and completing concussion paperwork. Updated Physical forms should be uploaded into the Dragonfly App or website at https://www.

Students without a physical will not be able to participate in workouts or tryouts.
Parents should plan to pick up their child on time each day at the back of the school beside the gym.
Tryouts are closed to parents/guardians, relative,s and friends.
